Before only two choices were available to the public – an abdominal hysterectomy or a vaginal hysterectomy. You may know that you are getting a vaginal hysterectomy, but there are still various procedures that you can choose from.

A Total Vaginal Hysterectomy, more commonly known as TVH, is a vaginal procedure which is achieved by making an incision through the vagina, not through the abdomen. The complete cervix and two ovaries are removed along with the cervix. If your surgeon needs to check for cancer cells or of scattering endometriosis, it is not recommended to have a Total Vaginal Hysterectomy. Additionally, if your uterus is enlarged to a certain degree, or in some cases, you have not given birth vaginally, then it is also not recommendable to get a TVH. There is potentially more loss of blood with this type of surgery.

A TVH has a rest period of 6-8 weeks, with lifting and straining not allowed for the entire duration. As well, you will have a restriction on intercourse for the whole of the initial recovery period. Hormone Replacement Therapy (HRT) may be recommended by your doctor as a consequence of the so-called “surgical menopause” caused by the removal of your ovaries.

A Partial Vaginal Hysterectomy (PVH) is a similar procedure but the ovaries and/or all or part of the cervix are not removed. Partial Vaginal Hysterectomy does not need immediate HRT since the ovaries are still there to give hormones. Partial Vaginal Hysterectomies relatively have the same recovery period as Total Vaginal Hysterectomies.

Laparoscopic Assisted Vaginal Hysterectomy – or LAVH, during which several small cuts are made in the abdominal wall through which slender metal tubes called “trocars” are inserted providing access for a laparoscope and other small surgical instruments. Cutting the uterus and the cervix is mostly done through the holes made by the trocars; the cervix and uterus are taken out through the vagina.

A Laparoscopic Assisted Vaginal Hysterectomy can be either partial or total; it is up to the doctor to decide whether to remove the ovaries and cervix along with the uterus. The need for HRT depends whether the procedure done was partial or total.

With a Laparoscopic Assisted Vaginal Hysterectomy, the recovery time is a little shorter than with the traditional Total Vaginal Hysterectomy. With a LAVH, you can expect 4 to 6 weeks of recovery, with lifting and straining restrictions for this whole period. Like the other kinds of vaginal hysterectomy there is also a restriction on having intercourse for the entire duration of the recovery period.
